Overview/Guidelines:
As long as the labeling and applicable reporting requirements are met, an Indiana practitioner may dispense any legend drug they are allowed to prescribe under their Indiana professional license; Controlled Substances Registration (CSR) and DEA registrations (if dispensing controlled substances); and/or collaborative practice agreement (if this is required to prescribe). Physicians who dispense controlled substances must register as a controlled substance practitioner.

A CSR may be issued to an applicant who submits the completed application.
